Employee BenefitsThe value of our benefits package we offer to our employee-owners (EO’s) has been recognized by the Washingtonian magazine. Full-time employee-owners of DECISIVE ANALYTICS Corporation (DAC) are eligible to receive the company’s comprehensive benefits package: Employee-Ownership - A preeminent aspect of our benefits is that we share ownership with our employee owners in recognition of the importance each individual is to our venture. Every EO will be rewarded for their contributions to the growth of the Company and share in its success through stock option grants in the forms of bonuses and profit sharing. 401K/Profit Sharing Plan - Further evidence of our commitment to our EO’s is the fact that our 401K/profit sharing plan is a plan with operates without the requirement for employee contribution and vests immediately. The Company will annually contribute a significant portion of each EO’s earned salary for the first calendar year of employment. Bonus Program - A bonus program is available to all EO’s based on exceptional contributions to the Company, with the amounts of the available bonus dependent on overall company performance. This plan includes spot bonuses, as well as year-end bonuses based upon the EO’s ability to meet stated goals and objectives, and overall contributions to the company’s growth. Comprehensive Health/Dental Insurance Coverage - A Comprehensive Medical/Vision Insurance Package provided through Blue Cross Blue Shield which the Company contributes 75% to the premiums. The Company’s premium contributions for medical and dental insurance include coverage for the EO, Spouse, Domestic Partners and eligible dependents. Flexible Spending Account (aka Cafeteria Plan) – DAC offers Flexible Spending Accounts, for medical payments, such as co-pays, eyeglasses, dental care, etc., and/or a Dependent Care Account for day care as permitted by IRS regulations. For those EOs who do not enroll in the BCBS plan and who obtain their own eligible health care coverage elsewhere DAC provides a Premium Reimbursement Account. Life/AD&D Insurance - EO’s life insurance equating to one times the employee’s salary - to a stated maximum - will be fully paid by the Company. Short & Long-Term Disability - EO’s will be covered by short and long term disability with maximum monthly benefit. Long Term Care Insurance – EO’s will be covered by long-term care insurance and EO’s may buy-up, dependent and extended family member options, paid at personal expense, may be available. Coverage depends upon approval by UNUM. Additional Life Insurance – EO’s and dependents may apply for Additional Life Insurance, paid at personal expense, through UNUM’s Lifestyle Protection Plan. Coverage depends upon approval by UNUM. Paid Leave – DAC holidays, sick days and personal/vacation days are included the total days of paid leave. Flex Time – EO’s are permitted, workload dependent, to work flexible daily and weekly schedules which are reconciled on a monthly basis. Education and Training – Reimbursement of tuition for approved educational courses and training, which are related to Company business objectives. Company-Sponsored Events - Every EO will have the opportunity to participate in numerous Company-sponsored events. Other - Other benefits include: paid parking or mass transit reimbursement, gym/health club membership subsidy, flexible work hours (with supervisor approval), selected attendance at company offsite meetings, discounts on select major sports events, advanced ticket sales on selected concerts/cultural events, as available.
